Post-Acute Care
The purpose of this task force is to build bridges, forge connections, between the various stakeholders that cater to the over 60 populations with services, products, housing and healthcare. This group is a forum for advocacy, communication and understanding amongst all segments of the health industry that affect the care of generations.
Goals:
- Improve communications between acute and post-acute care providers
- Establish a competency model by which hospitals and families can make informed choices on post-acute care providers (appropriate care settings backed by quality data, best practices, and clinical capabilities)
- Improve hand-off communication and medical management between hospitalists, primary care physicians, and post-acute care medical directors (multi-discipline accountability)
- Readmission Reduction protocols & tools for post-acute care providers…(rebound avoidance)
- Effective and Comprehensive Medication Reconciliation as part of communication to post-acute care providers
