Prevention Institute White Paper on Community Centered Health Homes
The Health Industry Council was privileged to co-host a presentation and process facilitation session with Larry Cohen, Founder of the Prevention Institute on Feb. 29th. A key take-away was his concept of community centered health homes. The nine organizations who collaborated to bring this event to DFW are making plans for future action items and events that keep prevention on the table in 2012.
